Change Attendance and Meals Form

Dear Parent,

If you wish to change the attendance for your child, or change the meals option, please complete and submit this form. All changes for attendance need a minimum of 2 weeks to be accepted. If changing meals, it is a terms notice. If changing from 38 weeks grant to 51 weeks stretched grant, we need a terms notice.

If changing from 51 weeks stretched to 38 weeks, please allow a full year to elapse to have the full benefit of the grant. Otherwise, you can change from the beginning of the following term, but you will lose grant hours.
